WORK ORDER AGREEMENT
Contract Number: 2072
Job Site Location: Tiburon Town Hall, 1505 Tiburon Boulevard, Tiburon, California.
This agreement is between Town Of Tiburon, hereinafter referred to as "Client", and
Scott Kern, California State Contractor's License Number: 483690, hereinafter referred
to as "Contractor". It is established for the purpose of providing Client with the
services described below. The parties agree as follows:
Goal:
Install cable and connections to support Voice services for twenty four jack locations,
within the Council Chambers, to support an interim Emergency Operations Center.
This proposal is based upon the information provided by Town Of Tiburon.

Workstation Cabling
Contractor will install one segment each, four pair Category 3 cable from the second
floor wire closet location to the two vacant outlet locations, within the Council
Chambers.
Workstation Termination
Within the Council Chambers, Contractor will remove and replace the existing
telephone faceplates and jacks so that the cables, including the two new cables, are
terminated to support four telephone jacks each (total of 24). All faceplates will be
labeled with each telephone number.
Wire Closet Termination
Contractor will terminate the two new cables onto 66M 1-50 punch blocks to match the
existing. Contractor will then cross-connect the 24 specified telephone numbers to the
newly installed jacks. If Client does not specify which numbers are assigned to which
jacks, than Contractor will randomly assign them.
Testing
Upon completion of the installation process, Contractor will test each jack.
Warranty
During the final planning stages and throughout the job, Contractor will confer with the
appointed representative(s) of Client on any point that is questionable. In addition,
Contractor will welcome any input that will help insure the total success of the job and
the complete satisfaction of Client. All work will be performed in a professional
manner and will be warranted against defects in material and workmanship for a period
of Three Years from the date of substantial completion.
