Configured with PRI and 23 stations. 8 Port 140-hour voicemail. 6 VIP stations. (19)ESI 30D-business phones and(3)ESI 55D Phones. (1)60 Button Console. Schedule of values: Total cost of Materials $10,737.50 Sales Tax $993.22 Freight $220.00 5 year Warranty $728.10 Total Labor $2,760.00 Total Cost of Town Hall $15,438.82 NOTES: • Lease Pricing is available upon request. • Any deviations from normal access to the work areas can result in delays,possible overtime,and additional costs. • Any changes to scope of work will result in a change order. • All conduit work to be done by others. • Pricing does not include cutting,patching,drilling,coring. • Pricing does not include rings,strings,sound pads,mud rings etc.unless stated in scope of work. • If J-hooks are not able to be shot,anchoring will be an additional charge. • Any unforeseen or undisclosed items in the above listed work will result in a change order. • Permit is not included in above pricing unless listed in schedule of values. If Permit is needed,please add$250.00 INSTALLATION REQUIREMENTS: • The Scope of Work outlined above reflects our estimate of the work to be performed based.on the information available. • Work will be done during normal working hours,except as described above.The work area must be accessible during the installation. CERTIFICATES OF INSURANCE: If your building requires certificates of insurance,please provide correct building information at least 7 days prior to start date of job. If this information is provided later,all attempts will be made to get the certificate sooner,but may impact start date of job. Our standard COIs come with"blanket"coverage"as required per written contract"for Waiver of Subrogation and Endorsements CG2010 and CG2037. Should any certificate holder want to have their names placed on the CG2010 and.CG2037,or should they require any additional endorsements,that will take up to four weeks to provide from the underwriter and will come at additional cost to the customer. ONGOING SUPPORT AFTER THIS PROPOSAL: IDT,Inc.can provide an additional agreement for on-going support for maintenance on your telephone system and can provide MAC support for your future needs. IDT,INC.WARRANTY: During the final planning stages and throughout the job,IDT,Inc.will confer with the appointed representative(s)of Client on any point that is questionable. In addition,IDT,Inc.will welcome any input that will help ensure the total success of the job and the complete satisfaction of Client. All work will be performed in a professional manner and will be warranted. Warrants all workmanship and materials installed in accordance with specifications and agrees to replace any work that may prove to be defective in workmanship or materials,within a period of one(1)year from the date of customer acceptance. Ordinary wear and tear,misuse,abuse,and neglect is excluded.This warranty will not apply to materials not provided by IDT,Inc. PAYMENT TERMS: IDT,Inc.will require 100%of the equipment cost including tax and shipping along with 50%of the labor as down payment upon signing of contract. Final payment is due upon customer acceptance.All Invoices are due upon receipt of invoice.All past due invoices will be charged 1.5%per annum on the unpaid balance with a monthly interest accrual.All change orders will be billed in progress billing. IDT,Inc.does accept credit card payments,a 2.5%charge will apply for Visa and a 3.5%charge will apply for Amex. If any information given is missing or incorrect there will be an additional charge of 2%added to your account. Please let IDT,Inc.know prior to the signing of the contract that you wish to pay via credit card. This proposal is good for 30 days. All Expedited Air Freight charges are not included. Materials in the above proposal are available in 7 to 10 days after receipt of order by normal shipping methods. The information in this quotation and.the quotation itself are proprietary in nature and shall not under any circumstance be released,in part or in whole to third parties without written approval of IDT,Inc.Any portion of this quotation released without written approval of IDT,Inc.will be billed to customer as consulting time at one hundred forty dollars($140.00)per hour with a minimum of 16 hours ATTORNEY'S FEES CLAUSE: In any action or arbitration arising out of this Agreement and pertaining to any cause of action,whether legal, equitable,contractual,or tortious,and relating to any damages,the prevailing party shall be entitled to reasonable attorney's fees, costs,and arbitration fees(if any). In determining the amount of such fees,the trier of fact shall consider the reasonableness of the fees in relation to the amount recovered and which party prevailed on the issues involved.
